Narrative:

Three aircraft on vectors for visual approachs to runway 4. Large transport X 11 mi ssw of airport cleared for approach. Medium large transport Y in trail from south with large transport X in sight was cleared first officer follow large transport X for runway 4. Small transport Z reported airport in sight 25 mi southwest and was proceeding straight in for runway 4. When small transport Z approximately 17 mi final, medium large transport Y traffic issued at 1 O'clock and 6 mi. Large transport X was 12 O'clock and 9 mi, traffic was not issued. Small transport Z reported traffic in sight and cleared to follow medium large transport Y. Medium large transport Y turning 10 mi final and slowing to follow large transport X. Small transport Z was not slowing down and traffic again issued, 12 O'clock and 3 mi, an medium large transport Y. Small transport Z again reported traffic in sight and advised he was slowing down. Small transport Z accepted landing runway 7 and told to maintain visual sep with the medium large transport Y and follow to the airport. Small transport Z continued to overtake medium large transport Y. When less than 1 mi in trail, small transport Z was vectored and given altitude to climb to. Traffic again issued. Small transport Z reported medium large transport Y in sight and said he had been following wrong aircraft. Pilots of the small transport Z have reputation for following traffic closely and keeping speed up. Controller felt small transport Z had right traffic in sight and was going to follow closely behind. Problem can be correct in 2 ways: first, pilot can't assume first aircraft he sees is the one he is supposed to follow. Second, both the controller and the pilot should indicate aircraft type when issuing and acknowledging traffic.

WHEN LESS THAN 1 MI IN TRAIL, SMT Z WAS VECTORED AND GIVEN ALT TO CLB TO. TFC AGAIN ISSUED. SMT Z RPTED MLG Y IN SIGHT AND SAID HE HAD BEEN FOLLOWING WRONG ACFT. PLTS OF THE SMT Z HAVE REPUTATION FOR FOLLOWING TFC CLOSELY AND KEEPING SPD UP. CTLR FELT SMT Z HAD RIGHT TFC IN SIGHT AND WAS GOING TO FOLLOW CLOSELY BEHIND. PROB CAN BE CORRECT IN 2 WAYS: FIRST, PLT CAN'T ASSUME FIRST ACFT HE SEES IS THE ONE HE IS SUPPOSED TO FOLLOW. SECOND, BOTH THE CTLR AND THE PLT SHOULD INDICATE ACFT TYPE WHEN ISSUING AND ACKNOWLEDGING TFC.
