Narrative:
During climb out; I had to declare an emergency to force a lateral deviation around a strong thunderstorm cell. I however; forgot to inform dispatch after arrival of this declaration as instructed the company operations manual. Since I have declared three emergencies over the last months for totally different reasons; I should have remembered this requirement even if this case of a far less exciting event than the others.

Title: A MD-80 Captain failed to file the Company required form after declaring an emergency while deviating for weather.
Narrative: During climb out; I had to declare an emergency to force a lateral deviation around a strong thunderstorm cell. I however; forgot to inform Dispatch after arrival of this declaration as instructed the Company Operations Manual.
