Narrative:
At the conclusion of a part-135 charter flight; the pilot had written two squawks: a leaking fuel sump drain and a cabin door locking issue. These were written on the aircrafts flight record sheet in the discrepancy column as required by our part 135 operations manual. This sheet was given to the director of maintenance (dom); who instructed a mechanic to fix the problems. The flight sheet was re-written; without the squawks and no log entry was prepared for the completed work. The consequences are: the pilot will not be aware of repairs made; proper repair station procedures were not followed; part 135/145 and far part 43.9 operations guidelines; for recording maintenance were not complied with.
Original NASA ASRS Text
Title: A Lead Technician with Inspection Authority (IA) reports about the unprofessional behavior of their Director of Maintenance (DOM) at their Part 135/145 helicopter charter and repair facility. The lack of any requirement to have pre-printed; sequentially numbered logbook pages; was considered to be a contributory factor by the IA.
