Narrative:
Aircraft operating with right pack off due to inoperative right prsov per MEL 16-01. During climbout at FL340; noticed cabin altitude climb rate increase to approximately 1000 ft/minute with pressurization outflow valve indicating full closed with normal left duct pressure. Requested lower altitude from ATC. Selected isol valve open and turned right pack on; at which point cabin altitude became controllable. Contacted dispatch for coordination of further flight and after conversing with flight attendants I was informed they and passengers reported some ear discomfort. Along with this physiological condition and uncertainty of the operating pack; an emergency was declared and coordinated with ATC and dispatch for return to our departure airport with descent to 1100 ft. Landed overweight at 204 pounds. No medical assistance required on arrival. Maintenance logbook entries completed.

Title: A B757-200 with an MEL'ed right engine Pressure Regulating Shutoff Valve and right pack OFF; began losing pressurization at FL340. An emergency was declared and the aircraft was returned to the departure airport
Narrative: Aircraft operating with right pack off due to inoperative right PRSOV per MEL 16-01. During climbout at FL340; noticed cabin altitude climb rate increase to approximately 1000 FT/minute with pressurization outflow valve indicating full closed with normal left duct pressure. Requested lower altitude from ATC. Selected ISOL valve open and turned right pack ON; at which point cabin altitude became controllable.
