Narrative:
Light transport Y at 10000', wbound, level. Medium large transport X at 15000' descending to 11000', reducing to 250K and read back 10000'. Controller did not hear readback. Aircraft passed 12 O'clock, 3 mi, with 1000'. Then, as mileage increased, altitude was lost.
