Narrative:
I was working radar while our supervisor was working local in the tower. A cessna departed; going out for practice approaches which neither the ground controller nor local controller confirmed. When the aircraft came over to me on departure; I turned him for an approach for ZZZ. Soon after the local controller said subject the cessna; can he get a turn for a departure coming out behind him. I gave him a heading that worked for me thinking that he knew I was going to turn the cessna; because he said subject the cessna which to me means he was going to provide visual. He then cleared the jet and never saw that I turned the guy towards ZZZ and turned the jet right into him. I believe what happened was a huge break down in communication. I'm not sure if there was any deal or not because I am pretty sure I had diverging courses.

Title: TRACON Controller described a loss of separation between two successive departures when the Tower Controller released the second aircraft into the path of the first; failed communications listed as the causal factor.
