Called in fatigued. I have been on the road for ten days due to back-to-back trips; 1 day off between trips and a pt [proficiency training]. My days off were taken for a pt; there was no time between trips to go home. I was in work mode for 11 days. The past 10 days I have been waking up early (5 to 6 am). I woke up about 6 am for an afternoon show. I was making errors and found myself to be slow in action and thought.
