Narrative:

A fuel valve was shipped. ZZZ 'received' [into stores inventory] said stock item number; finding out at that time; it was undeclared hazardous material. Said [fuel] valve was pulled from printer. It was pulled and shipped without my knowledge [as a lead stock clerk]. It was not entered into the hazmat checklist log; nor was it assigned to any stock clerk. It was shipped as a normal transfer. Hazmat procedures were not followed. To the best of my knowledge this is what occurred.
