Narrative:
I am making this statement in relation to the incident with forward cargo door; reported to me by company ramp service employee who was opening the door when the incident happened. Note: when the door was open; at about three quarters of the way [up]; the door [rotary] actuators failed; allowing door to slam down striking the aircraft on the door opening more than once. This incident could have killed anyone under the door.

Title: A Line Mechanic reports a B767-300 forward cargo door was approximately three-quarters of the way being opened when the door rotary actuators suddenly failed; allowing the door to slam down; striking the aircraft. The incident could have killed anyone under the door.
