Narrative:
On approach into mdw at 4000', being vectored, the controller then told me to maintain a lower altitude and to intercept the NDB runway 4R approach course. I rogered the clearance, but never reported leaving 4000'. The controller told me that I was 12 mi from cadon and cleared me for the approach. The approach plate for mdw specifies cross cadon at 4000', then descend. Unsure of my altitude, I stayed at 4000' until the controller started screaming that he wanted me down 'right now.' I was just outside of cadon. I was in conflict with an small transport. I dove the aircraft and the small transport climbed. We missed each other by a good distance. I then continued the approach from that point. I do think, if the controller wanted me down right then, he should have said so by stating, 'I need you out of 4000' now.' I should have verified the approach instructions, but the frequency was very busy. I should have read the fine print on the chart which said I could be outside of cadon at 2600' when authority/authorized by ATC.

Title: SMT WAS ISSUED A DESCENT, BUT MAINTAINED ALT AND CAUSED A CONFLICTION WITH ANOTHER ACFT.
Narrative: ON APCH INTO MDW AT 4000', BEING VECTORED, THE CTLR THEN TOLD ME TO MAINTAIN A LOWER ALT AND TO INTERCEPT THE NDB RWY 4R APCH COURSE. I ROGERED THE CLRNC, BUT NEVER RPTED LEAVING 4000'. THE CTLR TOLD ME THAT I WAS 12 MI FROM CADON AND CLRED ME FOR THE APCH. THE APCH PLATE FOR MDW SPECIFIES CROSS CADON AT 4000', THEN DSND. UNSURE OF MY ALT, I STAYED AT 4000' UNTIL THE CTLR STARTED SCREAMING THAT HE WANTED ME DOWN 'RIGHT NOW.' I WAS JUST OUTSIDE OF CADON. I WAS IN CONFLICT WITH AN SMT. I DOVE THE ACFT AND THE SMT CLBED. WE MISSED EACH OTHER BY A GOOD DISTANCE. I THEN CONTINUED THE APCH FROM THAT POINT. I DO THINK, IF THE CTLR WANTED ME DOWN RIGHT THEN, HE SHOULD HAVE SAID SO BY STATING, 'I NEED YOU OUT OF 4000' NOW.' I SHOULD HAVE VERIFIED THE APCH INSTRUCTIONS, BUT THE FREQ WAS VERY BUSY.
