Narrative:

Smoke in cockpit at FL360. Developed to heavy concentration in about 30 seconds originating under captain's seat. Masks on and immediately started a divert with emergency declared. Cabin prepared and was able to complete only first few pages of [the] smoke checklist before landing. Landed 16 minutes later. Smoke began to dissipate during descent; with only heavy electrical odor remaining during landing. Fire equipment inspected aircraft and followed to gate. Maintenance determined cockpit printer caught fire. Replaced faulty equipment and proceeded to destination.
