Narrative: SMT X WAS SEBND V23 FROM SAC CONFLICTED WITH ACR Y ARR AT 1,000' DUE TO WRONG ALT ASSIGNMENT. ACTION WAS TAKEN TO CORRECT, HOWEVER, LOSS OF SEPARATION DID OCCUR. (200' LATERAL.)
