Narrative:

On approach to st louis int'l airport, we failed to properly execute the lda 30L approach that we were assigned, possibly causing a traffic conflict with 30R. Initially we were told to expect the ILS approach to 30R. After the handoff to the final controller we were told to turn to our left and intercept the 30L lda. I acknowledged this instruction but the captain who was flying did not hear the change of apches. Therefore we did not make the left turn to intercept. The 2ND call from approach had us turn further left at which time I confirmed the change in the approach. At this time the captain responded by resetting his navigation radio to the ILS 30L frequency. It took a few more miles before I could convince him we were assigned the lda approach and not the ILS to 30L. At about 6000' and 10 mi from the airport, we broke out of the clouds and executed a VFR approach to 30L. Due to a crosswind component and the lateness in setting up for the lda approach, we never did get on the centerline of the approach. In a telephone call to the tower controller, no traffic conflict was mentioned.

Title: FLT CREW OF MLG NOT PROPERLY ORGANIZED BEFORE APCH, FAILS TO FLY APCH PROPERLY.

Narrative: ON APCH TO ST LOUIS INT'L ARPT, WE FAILED TO PROPERLY EXECUTE THE LDA 30L APCH THAT WE WERE ASSIGNED, POSSIBLY CAUSING A TFC CONFLICT WITH 30R. INITIALLY WE WERE TOLD TO EXPECT THE ILS APCH TO 30R. AFTER THE HANDOFF TO THE FINAL CTLR WE WERE TOLD TO TURN TO OUR LEFT AND INTERCEPT THE 30L LDA. I ACKNOWLEDGED THIS INSTRUCTION BUT THE CAPT WHO WAS FLYING DID NOT HEAR THE CHANGE OF APCHES. THEREFORE WE DID NOT MAKE THE LEFT TURN TO INTERCEPT. THE 2ND CALL FROM APCH HAD US TURN FURTHER LEFT AT WHICH TIME I CONFIRMED THE CHANGE IN THE APCH. AT THIS TIME THE CAPT RESPONDED BY RESETTING HIS NAVIGATION RADIO TO THE ILS 30L FREQ. IT TOOK A FEW MORE MILES BEFORE I COULD CONVINCE HIM WE WERE ASSIGNED THE LDA APCH AND NOT THE ILS TO 30L. AT ABOUT 6000' AND 10 MI FROM THE ARPT, WE BROKE OUT OF THE CLOUDS AND EXECUTED A VFR APCH TO 30L. DUE TO A XWIND COMPONENT AND THE LATENESS IN SETTING UP FOR THE LDA APCH, WE NEVER DID GET ON THE CENTERLINE OF THE APCH. IN A TELEPHONE CALL TO THE TWR CTLR, NO TFC CONFLICT WAS MENTIONED.
