Narrative:
Revision 402 dated 1/fri/90 contains ewr 3 departure (vector) dated 1/fri/90 (10-3). We had done this revision on 1/mon/90. When cleared to fly a ewr 2 departure, we discovered that the ewr 3 would not be effective until 1/thurs/90. We were forced to ask clearance for a readback of the ewr 2 departure, to confirm that we had the correct procedure. Both of us had inadvertently changed that page and destroyed the ewr 2. This mistake arose from the fact that revision letter 402-90 had 3 pages F index of changes with effective dates of 1/fri/90 and 1/mon/90, intermixed throughout. Upon discussion this with other pilots, we found that most had completed the entire change prior to 1/mon/90. The corrective action is simple but vital--that no revisions be issued with multiple effective dates. Either issue separate revisions, or coordinate the effective dates to be the same for a specific revision. At the very least, if multiple effective dates must be in the same revision, the index should list all of the pages for the first effective date alphabetically, before any of the later effective dates--thereby facilitating proper and timely changes.

Title: REPORTERS COMPLAIN OF PROC USED BY PUBLISHING COMPANY IN ISSUANCE OF MULTIPLE EFFECTIVE DATES IN ONE REVISION.
Narrative: REVISION 402 DATED 1/FRI/90 CONTAINS EWR 3 DEP (VECTOR) DATED 1/FRI/90 (10-3). WE HAD DONE THIS REVISION ON 1/MON/90. WHEN CLRED TO FLY A EWR 2 DEP, WE DISCOVERED THAT THE EWR 3 WOULD NOT BE EFFECTIVE UNTIL 1/THURS/90. WE WERE FORCED TO ASK CLRNC FOR A READBACK OF THE EWR 2 DEP, TO CONFIRM THAT WE HAD THE CORRECT PROC. BOTH OF US HAD INADVERTENTLY CHANGED THAT PAGE AND DESTROYED THE EWR 2. THIS MISTAKE AROSE FROM THE FACT THAT REVISION LETTER 402-90 HAD 3 PAGES F INDEX OF CHANGES WITH EFFECTIVE DATES OF 1/FRI/90 AND 1/MON/90, INTERMIXED THROUGHOUT. UPON DISCUSSION THIS WITH OTHER PLTS, WE FOUND THAT MOST HAD COMPLETED THE ENTIRE CHANGE PRIOR TO 1/MON/90. THE CORRECTIVE ACTION IS SIMPLE BUT VITAL--THAT NO REVISIONS BE ISSUED WITH MULTIPLE EFFECTIVE DATES. EITHER ISSUE SEPARATE REVISIONS, OR COORDINATE THE EFFECTIVE DATES TO BE THE SAME FOR A SPECIFIC REVISION. AT THE VERY LEAST, IF MULTIPLE EFFECTIVE DATES MUST BE IN THE SAME REVISION, THE INDEX SHOULD LIST ALL OF THE PAGES FOR THE FIRST EFFECTIVE DATE ALPHABETICALLY, BEFORE ANY OF THE LATER EFFECTIVE DATES--THEREBY FACILITATING PROPER AND TIMELY CHANGES.
