Narrative:
On initial push from lax indications of stuck hydraulic shut-off valve on #2 engine. Replaced valve and engine driven pump. Took off on normal flight and began losing hydraulic quantity in primary system. I flew the aircraft while captain and so executed appropriate checklists. Declared emergency and landed west/O incident at lax. I feel that proper maintenance procedures might have averted this problem. When performing the initial receiving aircraft checks almost all switches were in improper positions after returning from maintenance. This made me wonder if proper procedures were followed.

Title: ACR WDB EXPERIENCED LOSS OF HYDRAULIC QUANTITY. RETURNED TO DEP ARPT.
Narrative: ON INITIAL PUSH FROM LAX INDICATIONS OF STUCK HYD SHUT-OFF VALVE ON #2 ENG. REPLACED VALVE AND ENG DRIVEN PUMP. TOOK OFF ON NORMAL FLT AND BEGAN LOSING HYD QUANTITY IN PRIMARY SYS. I FLEW THE ACFT WHILE CAPT AND SO EXECUTED APPROPRIATE CHKLISTS. DECLARED EMER AND LANDED W/O INCIDENT AT LAX. I FEEL THAT PROPER MAINT PROCS MIGHT HAVE AVERTED THIS PROB. WHEN PERFORMING THE INITIAL RECEIVING ACFT CHKS ALMOST ALL SWITCHES WERE IN IMPROPER POSITIONS AFTER RETURNING FROM MAINT. THIS MADE ME WONDER IF PROPER PROCS WERE FOLLOWED.
