Narrative:
Crew had flown 9 hours 5 min with 12 hour 30 min on duty day prior. On this day crew was on 4TH leg with 6 hour flight time. 3 missed apches were made after radar apches to pns. Crew had 4 hours sleep night prior because of athletic team partying in hotel. Believe we were fatigued and this resulted in the inability to land from the apches. Believe a landing incident or accident could easily have resulted. #1: we should have requested release from duty rather than continue to fly. #2: new flight/time duty/times are marginal from safety standpoint.

Title: FLT CREW OF MLG MISSED 3 APCHES TO PNS WITH CEILING OF 400', ALLEGEDLY BECAUSE OF FATIGUE.
Narrative: CREW HAD FLOWN 9 HRS 5 MIN WITH 12 HR 30 MIN ON DUTY DAY PRIOR. ON THIS DAY CREW WAS ON 4TH LEG WITH 6 HR FLT TIME. 3 MISSED APCHES WERE MADE AFTER RADAR APCHES TO PNS. CREW HAD 4 HRS SLEEP NIGHT PRIOR BECAUSE OF ATHLETIC TEAM PARTYING IN HOTEL. BELIEVE WE WERE FATIGUED AND THIS RESULTED IN THE INABILITY TO LAND FROM THE APCHES. BELIEVE A LNDG INCIDENT OR ACCIDENT COULD EASILY HAVE RESULTED. #1: WE SHOULD HAVE REQUESTED RELEASE FROM DUTY RATHER THAN CONTINUE TO FLY. #2: NEW FLT/TIME DUTY/TIMES ARE MARGINAL FROM SAFETY STANDPOINT.
