Narrative:
After takeoff aircraft would not pressurize. Control of cabin pressure could not be regained by standby or manual backup portion of system. At approximately 6000' MSL the aft cargo door warning light illuminated. Flight returned to departure field west/O incident. This crew is certain we complied with all required checklist procedures to ascertain the door was closed prior to taxi from our gate. Also if the door light had been on a 'recall' system would have illuminated the amber master caution light when checked during the taxi/before takeoff checklist. The aft cargo door warning system was the subject of a recent company issued information bulletin. Apparently what occurred on our flight has happened previously. This bulletin states that mfr is redesigning the warning assembly. I am submitting this report to ensure there is a record of this problem and hope that some pressure can be exerted on mfr and the FAA to expedite the modification to the warning system.
Original NASA ASRS Text
Title: IN CLIMB REAR CARGO DOOR WARNING LIGHT COME ON PRESSURIZATION DIFFICULTIES FLT RETURNED AND LANDED.
Narrative: AFTER TKOF ACFT WOULD NOT PRESSURIZE. CTL OF CABIN PRESSURE COULD NOT BE REGAINED BY STANDBY OR MANUAL BACKUP PORTION OF SYS. AT APPROX 6000' MSL THE AFT CARGO DOOR WARNING LIGHT ILLUMINATED. FLT RETURNED TO DEP FIELD W/O INCIDENT. THIS CREW IS CERTAIN WE COMPLIED WITH ALL REQUIRED CHKLIST PROCS TO ASCERTAIN THE DOOR WAS CLOSED PRIOR TO TAXI FROM OUR GATE. ALSO IF THE DOOR LIGHT HAD BEEN ON A 'RECALL' SYS WOULD HAVE ILLUMINATED THE AMBER MASTER CAUTION LIGHT WHEN CHKED DURING THE TAXI/BEFORE TKOF CHKLIST. THE AFT CARGO DOOR WARNING SYS WAS THE SUBJECT OF A RECENT COMPANY ISSUED INFO BULLETIN. APPARENTLY WHAT OCCURRED ON OUR FLT HAS HAPPENED PREVIOUSLY. THIS BULLETIN STATES THAT MFR IS REDESIGNING THE WARNING ASSEMBLY. I AM SUBMITTING THIS RPT TO ENSURE THERE IS A RECORD OF THIS PROB AND HOPE THAT SOME PRESSURE CAN BE EXERTED ON MFR AND THE FAA TO EXPEDITE THE MODIFICATION TO THE WARNING SYS.
