Narrative:

About halfway back from ZZZZ I got a pretty bad headache followed by nausea. Brought the [international relief officer] back up off break and went and laid down in the bunk. Vomited twice over the next several hours and the first plan was for the first officer (first officer) to make the landing with me working the radio but as we approached ZZZ we all grew concerned about the possibility of me vomiting again while landing. Did vomit a third time in the descent. We decided that the best plan of action was for [the international relief officer] to be in the left seat for landing with me on the jump seat and the [other international relief officer] in the rt jump seat. Called dispatch and told him what was going on and he concurred so that is what we did. [The first officer] flew a flawless visual with [the international relief officer's] backing him up and then [he] taxied us right to the gate. The flight attendants were very helpful and did what they could for me and as far as I know the passengers never aware anything unusual was going on.I couldn't ask for a better crew.
