Narrative:
The plane was completely full. Passing ZZZZ I received a call from the back of the plane that a male had hit his head in the aft galley. Turns out he had mixed alcohol and ambien; causing him to pass out in the galley. The flight attendants tried to help him; but he wanted to fight. While talking to dispatch and medlink to see if there were any drugs to help him; his girlfriend with the same alcohol and ambien; vomited in the aft galley. While all this is happening the two aft fams subdued the male from seat xxy. Turns out he had urinated in the seat; on the floor and in the aft galley. Being so far north and getting ready to coast out on the atlantic crossing; medlink thought it best we land short and have this person check[ed] by a hospital because of the head injury. Medlink check[ed] ZZZZ for services; then felt it best to go to ZZZ; was 1.5 hours away and ZZZ1 was 2.5 hours away. Had this individual not hit his head we would have just continued on to ZZZZ1.with the help of dispatch we decided to divert to ZZZ. We had the third copilot come back to the cockpit to help with the diversion. Informed the passengers of what was happening. Ran the diversion checklist; fuel dump checklist; informed dispatch that when we land we were going to be fatigued and needed the plane crewed so that the passengers would be delayed as little as possible. This decision was not made lightly. We looked at the time we had been awake; the stressful situation and with diverting and the number of hours yet to fly with holding delays into ZZZZ1. After landing in gusty wind conditions we taxied to the gate where we were met by company personnel; police; and emergency personnel. After going to the hotel and sleeping very soundly because I was fatigued; I feel I made the right decisions.
