Narrative:
Aircraft had gone into maintenance due to a damaged elevator. Elevator was replaced and aircraft signed off. Pilot did a thorough preflight with special emphasis on the elevator. As per the checklist; trim and autopilot were verified. Trim was set for takeoff. On the takeoff roll the aircraft did an uncommanded rotation to an unsustainable climb angle. Pilot pushed the yoke forward and dialed in full down trim. With full down trim the aircraft was stabilized and an uneventful landing made.subsequent investigation showed that the trim had been misaligned by about 9 degrees. This was rectified and airplane returned to service.
