Narrative:

An medium large transport departed runway 9R climbing to 5000'. As he lifted an small aircraft I called northeast, southbound at 1500'. The aircraft was spotted during the call-up less than 1 mi north of the medium large transport's departure path and instructed to make an immediate right turn to the west for traffic, based on past observed medium large transport's climbout performance. There was no response, and traffic was issued to medium large transport. He replied he saw him and the small aircraft began his turn west. The medium large transport leveled his climb and passed beneath and perhaps 100 yds south of the small aircraft. When clear of traffic, the small aircraft was told to proceed on course and medium large transport was shipped to departure. About 2 mins later small aircraft asked if he 'could go back to patrick's frequency'? I think the situation occurred because someone didn't switch small aircraft to the correct frequency in a timely manner.

Title: AN SMA PASSING THROUGH THE ARPT TRAFFIC AREA CAME IN CLOSE PROX TO A DEP ACFT.

Narrative: AN MLG DEPARTED RWY 9R CLIMBING TO 5000'. AS HE LIFTED AN SMA I CALLED NE, SBND AT 1500'. THE ACFT WAS SPOTTED DURING THE CALL-UP LESS THAN 1 MI N OF THE MLG'S DEP PATH AND INSTRUCTED TO MAKE AN IMMEDIATE RIGHT TURN TO THE W FOR TFC, BASED ON PAST OBSERVED MLG'S CLIMBOUT PERFORMANCE. THERE WAS NO RESPONSE, AND TFC WAS ISSUED TO MLG. HE REPLIED HE SAW HIM AND THE SMA BEGAN HIS TURN W. THE MLG LEVELED HIS CLIMB AND PASSED BENEATH AND PERHAPS 100 YDS S OF THE SMA. WHEN CLEAR OF TFC, THE SMA WAS TOLD TO PROCEED ON COURSE AND MLG WAS SHIPPED TO DEP. ABOUT 2 MINS LATER SMA ASKED IF HE 'COULD GO BACK TO PATRICK'S FREQ'? I THINK THE SITUATION OCCURRED BECAUSE SOMEONE DIDN'T SWITCH SMA TO THE CORRECT FREQ IN A TIMELY MANNER.
