Narrative:
A departed air carrier mdt X was climbed to 4000'. Traffic was on the downwind at 4000' air carrier X should have been stopped at 3000' to ensure sep from air carrier Y on downwind at 4000'. The traffic got within 700' of each other before course divergence occurred. Cause: human error. Supplemental information from acn 142759: we departed clt on the way to flo with first officer at the controls. After takeoff clt tower instructed us to contact control. I made the call to departure, 'air carrier X out of 1700' for 3000'.' clt departure then instructed us to climb and maintain 4000'. Out of 3600' first officer called traffic at 12 O'clock. I looked and saw an air carrier mdt at 12 O'clock and what appeared to be level at 4000', which was our assigned altitude. I told clt that we had an air carrier at 12 O'clock. Clt departure then said that he thought we were level at 3000'. I told him that our last clearance was 4000', and that I had replied 4000'.

Title: A DEP ACR WAS INADVERTENTLY ISSUED AN ALT THAT CONFLICTED WITH ANOTHER ACR RESULTING IN LESS THAN STANDARD SEPARATION.
Narrative: A DEPARTED ACR MDT X WAS CLBED TO 4000'. TFC WAS ON THE DOWNWIND AT 4000' ACR X SHOULD HAVE BEEN STOPPED AT 3000' TO ENSURE SEP FROM ACR Y ON DOWNWIND AT 4000'. THE TFC GOT WITHIN 700' OF EACH OTHER BEFORE COURSE DIVERGENCE OCCURRED. CAUSE: HUMAN ERROR. SUPPLEMENTAL INFO FROM ACN 142759: WE DEPARTED CLT ON THE WAY TO FLO WITH F/O AT THE CONTROLS. AFTER TKOF CLT TWR INSTRUCTED US TO CONTACT CTL. I MADE THE CALL TO DEP, 'ACR X OUT OF 1700' FOR 3000'.' CLT DEP THEN INSTRUCTED US TO CLB AND MAINTAIN 4000'. OUT OF 3600' F/O CALLED TFC AT 12 O'CLOCK. I LOOKED AND SAW AN ACR MDT AT 12 O'CLOCK AND WHAT APPEARED TO BE LEVEL AT 4000', WHICH WAS OUR ASSIGNED ALT. I TOLD CLT THAT WE HAD AN ACR AT 12 O'CLOCK. CLT DEP THEN SAID THAT HE THOUGHT WE WERE LEVEL AT 3000'. I TOLD HIM THAT OUR LAST CLRNC WAS 4000', AND THAT I HAD REPLIED 4000'.
