Narrative:
2 flight plans issued. Wrong one used to insert in omega and plotting chart. Departed sju on cleared track. Later we declared position unknown and diverted to bermuda when our mistake was realized. Omega itself also experienced problems giving unrealistic and readouts of over 300 KTS. Fatigue a problem. 29 hours in 4 days. Supplemental information from acn 146899: 200 mi south of bda the captain and first officer concurred there was an anomaly with the ONS. Based on the inclement WX conditions at jfk, the ONS status and fuel status, a prudent decision was made to land in bda to rectify the situation.

Title: ACR LGT TRACK HEADING DEVIATION BETWEEN SJU AND JFK. THE FLT CREW HAD ENTERED THE WRONG FLT PLAN IN THE ONS NAVIGATION SYSTEM.
