Narrative:

A major communications failure was caused at den ARTCC when a new equipment was cut in during peak traffic with significant WX. Radio outages, radar outages, landline outages and computer interfacility outages affected the majority of sectors in den ARTCC. During the communications outages, the following unsafe, unsatisfactory conditions occurred: 1) unqualified personnel accepting handoffs over public phone lines. 2) numerous airspace deviations were not reported/processed. 3) pilots were told to call the next air traffic facility and identify themselves and provide flight plan information the next controller. 4) departures from den were not restr adequately. 5) overflts were not rerouted around affected sectors efficiently. 6) as many as 4 sectors were combined at 1 position during this period.

Title: THE COMS NETWORK BETWEEN CTLRS AND PLTS WAS INTERRUPTED WHEN NEW EQUIPMENT WAS BEING CUT OVER.

Narrative: A MAJOR COMS FAILURE WAS CAUSED AT DEN ARTCC WHEN A NEW EQUIP WAS CUT IN DURING PEAK TFC WITH SIGNIFICANT WX. DURING THE COMS OUTAGES, THE FOLLOWING UNSAFE, UNSATISFACTORY CONDITIONS OCCURRED: 1) UNQUALIFIED PERSONNEL ACCEPTING HDOFS OVER PUBLIC PHONE LINES. 2) NUMEROUS AIRSPACE DEVIATIONS WERE NOT RPTED/PROCESSED. 3) PLTS WERE TOLD TO CALL THE NEXT AIR TFC FAC AND IDENT THEMSELVES AND PROVIDE FLT PLAN INFO THE NEXT CTLR. 4) DEPS FROM DEN WERE NOT RESTR ADEQUATELY. 5) OVERFLTS WERE NOT REROUTED AROUND AFFECTED SECTORS EFFICIENTLY. 6) AS MANY AS 4 SECTORS WERE COMBINED AT 1 POS DURING THIS PERIOD.
