Narrative:
Shortly after start, flight attendant reported that although aft galley door was closed and latched, it was noisy. I checked door annunciator/lights, all indicated secure. Upon setting quiet EPR after takeoff, aircraft would not pressurize properly. While running loss of pressurization checklist, flight attendant reported aft galley door extremely noisy. I visly checked door and saw that part of the skirting at the base of the door was caught between door and frame. Returned to atlanta where maintenance inspected and properly closed the door. In retrospect, a better course of action on my part would have been to go aft and visly check the door before takeoff rather than accept the 'easy explanation' of a noisy door. Supplemental information from acn 152853. In spite of the fact that I had spent the last year as a crew resources management facilitor and some of that time directly spent encouraging flight attendants to come forward with information, I was not receptive. I allowed her low key and inassertive manner to underrate (discount) her information. Had I been really listening I would have inquired and checked further and would have easily resolved the problem prior to taxi. In fact the situation repeated itself 24 hours later with a more assertive flight attendant and with myself being more receptive, the problem was immediately and easily resolved.
Original NASA ASRS Text
Title: FA INDICATES A NOISY DOOR DURING TAXI. INDICATOR LIGHTS SHOW NO PROBLEM. UNABLE TO PRESSURIZE AFTER TKOF. RETURN LAND.
Narrative: SHORTLY AFTER START, FLT ATTENDANT RPTED THAT ALTHOUGH AFT GALLEY DOOR WAS CLOSED AND LATCHED, IT WAS NOISY. I CHKED DOOR ANNUNCIATOR/LIGHTS, ALL INDICATED SECURE. UPON SETTING QUIET EPR AFTER TKOF, ACFT WOULD NOT PRESSURIZE PROPERLY. WHILE RUNNING LOSS OF PRESSURIZATION CHKLIST, FLT ATTENDANT RPTED AFT GALLEY DOOR EXTREMELY NOISY. I VISLY CHKED DOOR AND SAW THAT PART OF THE SKIRTING AT THE BASE OF THE DOOR WAS CAUGHT BTWN DOOR AND FRAME. RETURNED TO ATLANTA WHERE MAINT INSPECTED AND PROPERLY CLOSED THE DOOR. IN RETROSPECT, A BETTER COURSE OF ACTION ON MY PART WOULD HAVE BEEN TO GO AFT AND VISLY CHK THE DOOR BEFORE TKOF RATHER THAN ACCEPT THE 'EASY EXPLANATION' OF A NOISY DOOR. SUPPLEMENTAL INFO FROM ACN 152853. IN SPITE OF THE FACT THAT I HAD SPENT THE LAST YEAR AS A CREW RESOURCES MGMNT FACILITOR AND SOME OF THAT TIME DIRECTLY SPENT ENCOURAGING FLT ATTENDANTS TO COME FORWARD WITH INFO, I WAS NOT RECEPTIVE. I ALLOWED HER LOW KEY AND INASSERTIVE MANNER TO UNDERRATE (DISCOUNT) HER INFO. HAD I BEEN REALLY LISTENING I WOULD HAVE INQUIRED AND CHKED FURTHER AND WOULD HAVE EASILY RESOLVED THE PROBLEM PRIOR TO TAXI. IN FACT THE SITUATION REPEATED ITSELF 24 HRS LATER WITH A MORE ASSERTIVE FLT ATTENDANT AND WITH MYSELF BEING MORE RECEPTIVE, THE PROB WAS IMMEDIATELY AND EASILY RESOLVED.
