Narrative:
We were given a descent to cross 25 mn of sax at 8000'. Captain was at controls, rogered clearance and proceeded to input the restriction into the FMC (flight management computer). Since no fix on the arwy matched the 25 mi restriction captain created one and inserted it into the flight plan. Since the first insertion of this information was not in the proper position in relation to existing fixes, it was reinserted in what we believed was the proper position. At that time the crew completed the preliminary landing checklist, discussed the ATIS and briefed the approach (ILS runway 4R circle to 29 at ewr). At approximately 31 mi north of sparta the first officer advised the captain that only 6 mi remained to the 8000' restriction. Aircraft was slowing from cruise descent speed to 250 KTS for descent below 10000'. (First officer had sax in his VOR for B/U). Captain disconnected autoplt deployed spdbrake, slowed to 250 KTS and descended as rapidly as possible. The 25 north restriction was missed and the aircraft leveled at 23.8 mi north at 8000'. Title: FLT CFEW OF MLG WAS CLEARED TO 8000' MSL 25 N OF SAX, NJ. FLT CREW PROGRAMMED THE ILL DEFINED FIX BUT MISLOCATED AND HAD TO REINSERT. OTHER COCKPIT TASKS AND BRIEFING DISTRACTED THE FLT CREW AND CROSSING RESTRICTION WAS NOT MET.
Narrative: WE WERE GIVEN A DSNT TO CROSS 25 MN OF SAX AT 8000'. CAPT WAS AT CTLS, ROGERED CLRNC AND PROCEEDED TO INPUT THE RESTRICTION INTO THE FMC (FLT MGMNT COMPUTER). SINCE NO FIX ON THE ARWY MATCHED THE 25 MI RESTRICTION CAPT CREATED ONE AND INSERTED IT INTO THE FLT PLAN. SINCE THE FIRST INSERTION OF THIS INFO WAS NOT IN THE PROPER POS IN RELATION TO EXISTING FIXES, IT WAS REINSERTED IN WHAT WE BELIEVED WAS THE PROPER POS. AT THAT TIME THE CREW COMPLETED THE PRELIMINARY LNDG CHKLIST, DISCUSSED THE ATIS AND BRIEFED THE APCH (ILS RWY 4R CIRCLE TO 29 AT EWR). AT APPROX 31 MI N OF SPARTA THE F/O ADVISED THE CAPT THAT ONLY 6 MI REMAINED TO THE 8000' RESTRICTION. ACFT WAS SLOWING FROM CRUISE DSNT SPD TO 250 KTS FOR DSNT BELOW 10000'.
