Narrative:
I was talking to my IFR student when we were given clearance to 'taxi into position and hold.' I did not hear the details as I was not paying enough attention. My student acknowledged and I thought included 'cleared for takeoff' in the acknowledgement. I remember becoming slightly impatient while he was setting his dg and several other pretkof runway items. I relaxed when he finally started his takeoff roll. Then the tower controller came on and sternly said that he did not clear us for takeoff, then asked if we had a military on final into NAS alameda in sight. We did. Then ordered visibility sep for us, which we did. I called the tower later to try and clear things up and they did not remember my student's exact acknowledgement, but did remember clearing us to taxi into position and hold only. They were pretty nice about it and told me not to worry about future action. I wasn't watching my student closely enough during a routine operation. He has some problems effecting him emotionally now, and is not up to his usual level--which is what I was expecting and using the required amount of attention for. I knew about his worries. Well, I should watch every student carefully in critical phases of flight--no exceptions. A CFI should be ready for goofy, as well as normal mistakes. I have had another problem with this student recently because of just not expecting him to slip so much. Not again--I'm still learning as a cfii, mostly on how to handle students now.

Title: INSTRUCTOR AND STUDENT PLT CLEARED POSITION AND HOLD, BUT STARTED TKOF ROLL.
Narrative: I WAS TALKING TO MY IFR STUDENT WHEN WE WERE GIVEN CLRNC TO 'TAXI INTO POS AND HOLD.' I DID NOT HEAR THE DETAILS AS I WAS NOT PAYING ENOUGH ATTN.
