Narrative:
IFR flight, buf to ith, night and heavy fog in ramp area. After copilot read back our clearance, I, turned ATIS on again, to confirm that we still had takeoff minimums. ATIS was reporting runway 23 in use and no mention of runway 14. I did not request runway 14. In the hurried atmosphere and monitoring two frequencys, I just did not hear ground control say 'runway 14'. I had told the copilot that I was familiar, so she told 'ground' we didn't need 'progressive taxi'. I taxied south on taxiway quebec and xed runway 14/32, then left on 'delta'. The patchy ice and fog made progress slow and only because I was familiar I located the hold line short of runway 23/05. After a couple mins, I reported holding short, to cross runway 23. The controller asked for a repeat of my position. We were soon cleared to cross and taxi to runway 23. We took off with a 2200' RVR and midpoint RVR reported as 1100'. The incident happened, I think, due to several factors. Hurrying, as WX was deteriorating, and poor coordination between crew members, (due to hurried atmosphere). This incident could have been prevented by an installation of a red/green light in the taxiway at the hold line. Otherwise, controllers should not assign another runway unless requested.

Title: GA SMT TAXIED TO WRONG RWY CROSS THE ACTIVE RWY WITHOUT CLRNC.
Narrative: IFR FLT, BUF TO ITH, NIGHT AND HVY FOG IN RAMP AREA. AFTER COPLT READ BACK OUR CLRNC, I, TURNED ATIS ON AGAIN, TO CONFIRM THAT WE STILL HAD TKOF MINIMUMS. ATIS WAS RPTING RWY 23 IN USE AND NO MENTION OF RWY 14. I DID NOT REQUEST RWY 14. IN THE HURRIED ATMOSPHERE AND MONITORING TWO FREQS, I JUST DID NOT HEAR GND CTL SAY 'RWY 14'. I HAD TOLD THE COPLT THAT I WAS FAMILIAR, SO SHE TOLD 'GND' WE DIDN'T NEED 'PROGRESSIVE TAXI'. I TAXIED S ON TXWY QUEBEC AND XED RWY 14/32, THEN L ON 'DELTA'. THE PATCHY ICE AND FOG MADE PROGRESS SLOW AND ONLY BECAUSE I WAS FAMILIAR I LOCATED THE HOLD LINE SHORT OF RWY 23/05. AFTER A COUPLE MINS, I RPTED HOLDING SHORT, TO CROSS RWY 23. THE CTLR ASKED FOR A REPEAT OF MY POS. WE WERE SOON CLRED TO CROSS AND TAXI TO RWY 23. WE TOOK OFF WITH A 2200' RVR AND MIDPOINT RVR RPTED AS 1100'. THE INCIDENT HAPPENED, I THINK, DUE TO SEVERAL FACTORS. HURRYING, AS WX WAS DETERIORATING, AND POOR COORD BTWN CREW MEMBERS, (DUE TO HURRIED ATMOSPHERE). THIS INCIDENT COULD HAVE BEEN PREVENTED BY AN INSTALLATION OF A RED/GREEN LIGHT IN THE TXWY AT THE HOLD LINE. OTHERWISE, CTLRS SHOULD NOT ASSIGN ANOTHER RWY UNLESS REQUESTED.
