Narrative:
On a flight from ads to tki, an IFR flight plan was filed to tki with ads as an alternate. A VOR/DME approach was made into tki west/O DME on board the aircraft. Callback conversation with reporter revealed the following information. Reporter states not sure how FAA learned of the violation. Believes someone on the field at another flight school reported. FAA has requested to see rptrs logbook. Reporter believes they are looking at the incident date. Does not know for sure. Says should have cancelled IFR as they were VFR at the time and have the duat printout to verify. Callback conversation with reporter, from supplemental acn 175154, revealed the following information. Reporter statement much the same as 175155 rptrs. They were together when they called ASRS. In addition he adds that he has flown the VOR DME approach many times before and knows the position of the DME fixes. But agrees should have cancelled and continued VFR.

CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO. RPTR STATES NOT SURE HOW FAA LEARNED OF THE VIOLATION. BELIEVES SOMEONE ON THE FIELD AT ANOTHER FLT SCHOOL RPTED. FAA HAS REQUESTED TO SEE RPTRS LOGBOOK. RPTR BELIEVES THEY ARE LOOKING AT THE INCIDENT DATE. DOES NOT KNOW FOR SURE. SAYS SHOULD HAVE CANCELLED IFR AS THEY WERE VFR AT THE TIME AND HAVE THE DUAT PRINTOUT TO VERIFY. CALLBACK CONVERSATION WITH RPTR, FROM SUPPLEMENTAL ACN 175154, REVEALED THE FOLLOWING INFO. RPTR STATEMENT MUCH THE SAME AS 175155 RPTRS. THEY WERE TOGETHER WHEN THEY CALLED ASRS. IN ADDITION HE ADDS THAT HE HAS FLOWN THE VOR DME APCH MANY TIMES BEFORE AND KNOWS THE POSITION OF THE DME FIXES. BUT AGREES SHOULD HAVE CANCELLED AND CONTINUED VFR.
