Narrative:

Was cleared by dispatch release to fly as airlines flight XXX from stl to houston hobby with 12000 pounds of fuel. Approaching little rock it was observed that the fuel was down to about 7000 pounds and we were flying against very strong headwinds (300 KTS ground speed). It was decided that insufficient fuel remained to continue the flight with sufficient reserve in houston. The flight was diverted to light transport. The aircraft was refueled and the flight was continued to houston west/O further problems. As arises often in commercial aviation, the repetitive nature is a pitfall for taking for granted that all parties have not erred in the conduct of their duties. It is incumbent for all parties, dispatchers, operations agents, capts, first officer's, F/east's to be vigilant of all aspects of flts to ensure there are no breakdowns, as was the case in this incident. Supplemental information from acn 177606: the problem started with the release form (which the captain is required to sign prior to each flight) that stated a 6.3 burnoff and a flight plan that showed a 10.9 burnoff.

Title: ACR MLG DEPARTED STL FOR HOU WITHOUT SUFFICIENT FUEL AND HAD TO DIVERT TO STL AND REFUEL.

Narrative: WAS CLRED BY DISPATCH RELEASE TO FLY AS AIRLINES FLT XXX FROM STL TO HOUSTON HOBBY WITH 12000 LBS OF FUEL. APCHING LITTLE ROCK IT WAS OBSERVED THAT THE FUEL WAS DOWN TO ABOUT 7000 LBS AND WE WERE FLYING AGAINST VERY STRONG HEADWINDS (300 KTS GND SPD). IT WAS DECIDED THAT INSUFFICIENT FUEL REMAINED TO CONTINUE THE FLT WITH SUFFICIENT RESERVE IN HOUSTON. THE FLT WAS DIVERTED TO LTT. THE ACFT WAS REFUELED AND THE FLT WAS CONTINUED TO HOUSTON W/O FURTHER PROBS. AS ARISES OFTEN IN COMMERCIAL AVIATION, THE REPETITIVE NATURE IS A PITFALL FOR TAKING FOR GRANTED THAT ALL PARTIES HAVE NOT ERRED IN THE CONDUCT OF THEIR DUTIES. IT IS INCUMBENT FOR ALL PARTIES, DISPATCHERS, OPS AGENTS, CAPTS, F/O'S, F/E'S TO BE VIGILANT OF ALL ASPECTS OF FLTS TO ENSURE THERE ARE NO BREAKDOWNS, AS WAS THE CASE IN THIS INCIDENT.
