Narrative:

Factors affecting the quality of human performance: I was dispatched by company dispatch requiring 10.8 hours flight and 16.8 hours of duty time under part 135 in a 24 hours period. I lost power on the right engine after 10.0 hours of flight time and my performance was not up to PAR. I had a medical patient on board in critical condition, which affected my decision to continue beyond normal duty and flight times. A company dispatch decision needs to be corrected and more activate with patients on board. This happened at other times over the last 2 yrs. I am working with the chief pilot to help correct the problem.
