Narrative:
Crossing dotty intersection, we were cleared NA148 to miami. We then proceeded direct topps intersection. Approximately 150 mi past dotty, gander center questioned our routing, indicating his understanding was that NA148 was dotty to tusky. Upon closer evaluation, we realized that our latest commercial chart revision was not effective until many days later. It appears that there is ample room for error when revisions are published with different effective dates, especially when distributed in advance of future revisions. One could easily revise their manual prematurely, such as in our case, or lose the leftover pages of a revision effective on a later date. The fact that the captain, the first officer and I all fell into the trap is testimony to this.
