Narrative:
During initial preflight, first officer set altitude to 5000' (normally reset altitude upon receipt of clearance). Received clearance via ACARS and compared ACARS clearance to flight plan. First officer stated, 'as filed.' 'did not review' 8000' or hornet 3 departure, haray transition procedures and did not reset altitude to 8000'. Upon departure flew runway heading 'did not' turn to 330 degrees as specified by hornet 3. Leveled off 5000' instead of 8000'. Controller stated, 'do you have the hornet 3 departure.' responded yes. Frantically looked at hornet 3. Controller: 'climb to 8000'. Do you have hornet 3 departure?' responded yes, and dramatically looked at hornet 3 again. Controller: 'climb to 12000'.' at 12000' controller: 'what is your speed?' responded '300 KIAS.' controller: 'slow to 250 KIAS and call clt departure control on telephone upon landing.' hornet 3 states 250 KIAS until advised by ATC. At level off captain and I reviewed what had happened--it was scary! Captain called controller upon landing dfw.

LEVELED OFF 5000' INSTEAD OF 8000'. CTLR STATED, 'DO YOU HAVE THE HORNET 3 DEP.' RESPONDED YES. FRANTICALLY LOOKED AT HORNET 3. CTLR: 'CLB TO 8000'. DO YOU HAVE HORNET 3 DEP?' RESPONDED YES, AND DRAMATICALLY LOOKED AT HORNET 3 AGAIN. CTLR: 'CLB TO 12000'.' AT 12000' CTLR: 'WHAT IS YOUR SPD?' RESPONDED '300 KIAS.'
