Air Traffic Incident | Operational Deviation Intra Facility Coordination Failure Inter Facility Coordination Failure |
Narrative:
ARTS interface failure. Center handed small aircraft off to wrong sector. Training was being conducted on position which should have received the handoff. Position which received handoff then made a handoff to the correct position physical point to the trainee. Fpl responsible for position never heard the original handoff or observed the physical handoff, thus no knowledge of the small aircraft. Adjacent center called inquired if I knew an aircraft on beacon XXXX. It was the handoff the other position had taken, small aircraft involved was 7 mi inside the MOA. Automation did not tag the small aircraft also another aircraft with same beacon code had tagged as the small aircraft and was at FL220 nwbound when the small aircraft was at 5000 and was nebound. Controller called the deviation through his supervisor. Incident occurred at boundary of approach, jax center and atl center, poor radar coverage poor coordination within approach control facility, bad judgement, presently decertified.

Title: TRACON ARTCC COMPUTER INTERFACE PROBLEM LEADS TO INTERFAC INTRAFAC COORD INCIDENT THAT ALLOWED AN SMA TO ENTER A MOA WITHOUT CLRNC.
Narrative: ARTS INTERFACE FAILURE. CENTER HANDED SMA OFF TO WRONG SECTOR. TRAINING WAS BEING CONDUCTED ON POS WHICH SHOULD HAVE RECEIVED THE HDOF. POS WHICH RECEIVED HDOF THEN MADE A HDOF TO THE CORRECT POS PHYSICAL POINT TO THE TRAINEE. FPL RESPONSIBLE FOR POS NEVER HEARD THE ORIGINAL HDOF OR OBSERVED THE PHYSICAL HDOF, THUS NO KNOWLEDGE OF THE SMA. ADJACENT CENTER CALLED INQUIRED IF I KNEW AN ACFT ON BEACON XXXX. IT WAS THE HDOF THE OTHER POS HAD TAKEN, SMA INVOLVED WAS 7 MI INSIDE THE MOA. AUTOMATION DID NOT TAG THE SMA ALSO ANOTHER ACFT WITH SAME BEACON CODE HAD TAGGED AS THE SMA AND WAS AT FL220 NWBOUND WHEN THE SMA WAS AT 5000 AND WAS NEBOUND. CTLR CALLED THE DEV THROUGH HIS SUPVR. INCIDENT OCCURRED AT BOUNDARY OF APCH, JAX CENTER AND ATL CENTER, POOR RADAR COVERAGE POOR COORD WITHIN APCH CTL FACILITY, BAD JUDGEMENT, PRESENTLY DECERTIFIED.
