Narrative:
Printed departure clearance required mountain 2 SID departure. Captain flew runway heading, failed to turn as per SID. No mention by departure control. Clearance given to turn and proceed direct via filed routing. Departure SID very obscure as printed on printed departure clearance. At very bottom of printed copy should maybe be included in routing section like most flight plans. We just failed to note it during the planning and briefing!

DEP SID VERY OBSCURE AS PRINTED ON PRINTED DEP CLRNC. AT VERY BOTTOM OF PRINTED COPY SHOULD MAYBE BE INCLUDED IN RTING SECTION LIKE MOST FLT PLANS.
