Narrative:
Flight pushed off gate after contacting ramp control. When ready to taxi we contacted gate hold on 125.05 as we were instructed to do by clearance delivery. Gate hold told us no delays and I understood them to say taxi out fr on the inner. We followed these instructions and were told to monitor ground 121.9. We taxied down the inner and were in the vicinity of H taxiway. As we contacted ground control they asked us who cleared us down the inner. I responded the guy on 125.05. Ground control said this was the first he heard of it. The captain stopped the aircraft and asked for further instructions. We were cleared 2 left turns and taxi back on the outer. We made the turns at J thinking we were being sent to 22R which along with 31L was being used for takeoff. At this point ground control told us they wanted us to make the left turns at ma not J. We finally got all the communication straight, made a left at H, left on the inner and taxied to kk for takeoff. Factors that contributed included hustling to make an on- time departure making an unscheduled aircraft swap, complicated ATC procedures and being told to monitor ground control instead of making positive contact.

Title: FLC OF MLG TAXIED ONTO TAXIWAY WITHOUT COMMUNICATING WITH GND CTLR.
Narrative: FLT PUSHED OFF GATE AFTER CONTACTING RAMP CTL. WHEN READY TO TAXI WE CONTACTED GATE HOLD ON 125.05 AS WE WERE INSTRUCTED TO DO BY CLRNC DELIVERY. GATE HOLD TOLD US NO DELAYS AND I UNDERSTOOD THEM TO SAY TAXI OUT FR ON THE INNER. WE FOLLOWED THESE INSTRUCTIONS AND WERE TOLD TO MONITOR GND 121.9. WE TAXIED DOWN THE INNER AND WERE IN THE VICINITY OF H TAXIWAY. AS WE CONTACTED GND CTL THEY ASKED US WHO CLRED US DOWN THE INNER. I RESPONDED THE GUY ON 125.05. WE FINALLY GOT ALL THE COM STRAIGHT, MADE A L AT H, L ON THE INNER AND TAXIED TO KK FOR TKOF.
