Narrative:
Upon takeoff roll aircraft lost #4 engine cowl. During the day we had cowls open on all the engines. We closed them and they all appeared to be latched and secure. Supplemental information from acn 199900. I arrived after work was completed, performed walk around, signed air worthiness release and dispatched aircraft.

Narrative: UPON TKOF ROLL ACFT LOST #4 ENG COWL. DURING THE DAY WE HAD COWLS OPEN ON ALL THE ENGS. WE CLOSED THEM AND THEY ALL APPEARED TO BE LATCHED AND SECURE. SUPPLEMENTAL INFO FROM ACN 199900. I ARRIVED AFTER WORK WAS COMPLETED, PERFORMED WALK AROUND, SIGNED AIR WORTHINESS RELEASE AND DISPATCHED ACFT.
