Narrative:
When filling out medical application I included military flight time in total before noticing the 'civil'. I had intended to ask the doctor the proper way to make a correction on the form but forgot and the form was sent with the time not corrected. This was just an honest mistake in paperwork but with all the noise they are making about the medical applications these days it seemed wise to report it.

Title: MIL PLT FILLS OUT MEDICAL FORM FOR PHYSICAL WITH MIL FLT HRS INSTEAD OF CIVIL FLT HRS.
Narrative: WHEN FILLING OUT MEDICAL APPLICATION I INCLUDED MIL FLT TIME IN TOTAL BEFORE NOTICING THE 'CIVIL'. I HAD INTENDED TO ASK THE DOCTOR THE PROPER WAY TO MAKE A CORRECTION ON THE FORM BUT FORGOT AND THE FORM WAS SENT WITH THE TIME NOT CORRECTED.
