Narrative:

Departed ewr on newark 4 SID with a paper 'pre-departure clearance' to fly such SID off runway 22R. Climbing through 2500 ft, I contacted departure control. He said identify and I did so. Then he queried my altitude clearance/climbing call of '2500 ft climbing to 5000 ft.' departure control said we should have been held down to 2500 ft, but to maintain 5000 ft anyway. He changed squawk from XXX to YYY. Turns out that our agent pulled 'pre-departure clearance' out of computer for mar/xz instead of this day mar/yz. This day-old 'pre-departure clearance' had mar/yz printed on it 3 places, and mar/xz printed on it only once -- the operative place, of course.
