Narrative:
Cleared the STAR from the west into mem. About 5 mi west, approach asked if we were in the left turn. I said no, and we were directed to turn to 355 degrees. We had both failed to note the turn to the north at twikl intersection. Approach advised us that the STAR had changed several months before, and now when they're landing 18, you turn north at 8 DME (twikl) as depicted. This was all precipitated by a cockpit distraction. The captain finished his meal about 50 mi out at FL220 or so and put his tray on the jumpseat (folded), and pushed the flight attendant call button to have it picked up. No one responded and, about 3-4 mins later, the tray fell on the floor. The captain got up to pick up the mess and take it back to the galley, and while he was gone I got real busy with several radio calls and altitude changes. Having to handle the radios and altitude changes, etc, I got distracted and failed to notice that the arrival had changed since the last time I flew it. Ditto for the captain.
Original NASA ASRS Text
Title: AN MLG ACR CREW HAD A HDG DEV WHILE IN A TCA ON APCH, BOTH PLTS FAILED TO NOTE THE CHANGES IN THE NEW STAR, BOTH PLTS WERE DISTRACTED, AND VALUABLE TIME WAS LOST, BY A MEAL TRAY THAT HAD NOT BEEN PICKED UP BY THE FLT ATTENDANTS.
Narrative: CLRED THE STAR FROM THE W INTO MEM. ABOUT 5 MI W, APCH ASKED IF WE WERE IN THE L TURN. I SAID NO, AND WE WERE DIRECTED TO TURN TO 355 DEGS. WE HAD BOTH FAILED TO NOTE THE TURN TO THE N AT TWIKL INTXN. APCH ADVISED US THAT THE STAR HAD CHANGED SEVERAL MONTHS BEFORE, AND NOW WHEN THEY'RE LNDG 18, YOU TURN N AT 8 DME (TWIKL) AS DEPICTED. THIS WAS ALL PRECIPITATED BY A COCKPIT DISTR. THE CAPT FINISHED HIS MEAL ABOUT 50 MI OUT AT FL220 OR SO AND PUT HIS TRAY ON THE JUMPSEAT (FOLDED), AND PUSHED THE FLT ATTENDANT CALL BUTTON TO HAVE IT PICKED UP. NO ONE RESPONDED AND, ABOUT 3-4 MINS LATER, THE TRAY FELL ON THE FLOOR. THE CAPT GOT UP TO PICK UP THE MESS AND TAKE IT BACK TO THE GALLEY, AND WHILE HE WAS GONE I GOT REAL BUSY WITH SEVERAL RADIO CALLS AND ALT CHANGES. HAVING TO HANDLE THE RADIOS AND ALT CHANGES, ETC, I GOT DISTRACTED AND FAILED TO NOTICE THAT THE ARR HAD CHANGED SINCE THE LAST TIME I FLEW IT. DITTO FOR THE CAPT.
