Narrative:
Air carrier X on 5 mi final (visual approach) to runway 23L at cle, after having been cleared to land, the cle tower controller advised us that we had traffic at 11 O'clock and 1 mi. We were in a descent out of approximately 2500 MSL. We were unable to visually spot the traffic. The first officer asked the altitude of the traffic and was told it was an small aircraft Y at 2000 ft. Controller also told us to maintain 2500. We were about 100 ft to 150 ft below 2500 and began reversing descent and climbing. In the next 10 seconds TCASII issued a TA indicating target directly below us 300 ft below our altitude. Next came an RA commanding a climb of 1500-2000 FPM. Obeyed RA command and made a go around. Later (on the ground) I spoke with ATC supervisor on telephone. He told me approach control had instructed the small aircraft Y to make a left base entry for runway 23R. Approach control forgot to switch small aircraft Y to tower and forgot he overlooked handoff. Tower controller did not advise us that he was not talking to small aircraft Y.

Title: APCH CTLR FORGOT SMA Y. ACR X TCASII TA RA CLB HAD NMAC LTSS FROM SMA Y. EVASIVE ACTION TAKEN. SYS ERROR. TRACON ATCT INTERFAC COORD.
Narrative: ACR X ON 5 MI FINAL (VISUAL APCH) TO RWY 23L AT CLE, AFTER HAVING BEEN CLRED TO LAND, THE CLE TWR CTLR ADVISED US THAT WE HAD TFC AT 11 O'CLOCK AND 1 MI. WE WERE IN A DSCNT OUT OF APPROX 2500 MSL. WE WERE UNABLE TO VISUALLY SPOT THE TFC. THE FO ASKED THE ALT OF THE TFC AND WAS TOLD IT WAS AN SMA Y AT 2000 FT. CTLR ALSO TOLD US TO MAINTAIN 2500. WE WERE ABOUT 100 FT TO 150 FT BELOW 2500 AND BEGAN REVERSING DSCNT AND CLBING. IN THE NEXT 10 SECONDS TCASII ISSUED A TA INDICATING TARGET DIRECTLY BELOW US 300 FT BELOW OUR ALT.
