Narrative:
I picked up small aircraft aircraft from annual inspection aug/thu/92. Did extensive preflight and flew plane from lwm to psm. Following morning, again preflted and flew to lwm. Gear would not retract but green light went out and I could not get it back on even after going through emergency gear extension procedure. I could see, through a mirror on engine cowling, that the nose wheel appeared down, and though I pulled the circuit relay and manually attempted to turn handle to lock gear, I was unable to get green light even after resetting circuit relay and checking circuit breaker. I informed lawrence tower of my problem and did a fly-by to which the controller said the gear 'appeared down and locked.' my passenger (my wife) who is also a pilot and has been trained in emergency procedures, also tried extending the gear again, as did I before the fly-by. Upon landing the left gear folded. No injury and minimal damage. To date, no information as to what caused gear to fail and why we couldn't extend it manually to lock.

Title: SMA ACFT LNDG GEAR COLLAPSED DURING LNDG.
Narrative: I PICKED UP SMA ACFT FROM ANNUAL INSPECTION AUG/THU/92. DID EXTENSIVE PREFLT AND FLEW PLANE FROM LWM TO PSM. FOLLOWING MORNING, AGAIN PREFLTED AND FLEW TO LWM. GEAR WOULD NOT RETRACT BUT GREEN LIGHT WENT OUT AND I COULD NOT GET IT BACK ON EVEN AFTER GOING THROUGH EMER GEAR EXTENSION PROC. I COULD SEE, THROUGH A MIRROR ON ENG COWLING, THAT THE NOSE WHEEL APPEARED DOWN, AND THOUGH I PULLED THE CIRCUIT RELAY AND MANUALLY ATTEMPTED TO TURN HANDLE TO LOCK GEAR, I WAS UNABLE TO GET GREEN LIGHT EVEN AFTER RESETTING CIRCUIT RELAY AND CHKING CIRCUIT BREAKER. I INFORMED LAWRENCE TWR OF MY PROBLEM AND DID A FLY-BY TO WHICH THE CTLR SAID THE GEAR 'APPEARED DOWN AND LOCKED.' MY PAX (MY WIFE) WHO IS ALSO A PLT AND HAS BEEN TRAINED IN EMER PROCS, ALSO TRIED EXTENDING THE GEAR AGAIN, AS DID I BEFORE THE FLY-BY. UPON LNDG THE L GEAR FOLDED. NO INJURY AND MINIMAL DAMAGE.
