Narrative:
Got type rating on ATP certificate on nov/tue/92. Temporary certificate expired 120 days later. Prior to expiration of temporary certificate, I had extension faxed to me on mar/fri/93. 2 days later while en route on trip, I discovered that extension to the temporary to the permanent had expired. Original permanent certificate was taken from me at time of temporary issuance. On previous pilot certificates, I have always listed a post office box as a permanent address. At the air carrier inspector's insistance, a post office box and street number and name were listed as my mailing address. I was later informed by the postal service that dual addressed mail is returned to sender. Before temporary expired, I contacted FAA to correct the address problem. I was told address correction was made at the time my extension was faxed to me. More than 60 days have passed since correction was supposedly made by FAA, and I still do not have my permanent certificate. Callback conversation with reporter revealed the following information: reporter states he made many more calls to okc, received facsimile with extension of temporary certificate again. Finally received permanent copy.

Title: MLG FO FLYING WITH EXPIRED TEMPORARY CERTIFICATE. ADDRESS MIXUP AND PERMANENT NOT RECEIVED.
Narrative: GOT TYPE RATING ON ATP CERTIFICATE ON NOV/TUE/92. TEMPORARY CERTIFICATE EXPIRED 120 DAYS LATER. PRIOR TO EXPIRATION OF TEMPORARY CERTIFICATE, I HAD EXTENSION FAXED TO ME ON MAR/FRI/93. 2 DAYS LATER WHILE ENRTE ON TRIP, I DISCOVERED THAT EXTENSION TO THE TEMPORARY TO THE PERMANENT HAD EXPIRED. ORIGINAL PERMANENT CERTIFICATE WAS TAKEN FROM ME AT TIME OF TEMPORARY ISSUANCE. ON PREVIOUS PLT CERTIFICATES, I HAVE ALWAYS LISTED A POST OFFICE BOX AS A PERMANENT ADDRESS. AT THE ACR INSPECTOR'S INSISTANCE, A POST OFFICE BOX AND STREET NUMBER AND NAME WERE LISTED AS MY MAILING ADDRESS. I WAS LATER INFORMED BY THE POSTAL SVC THAT DUAL ADDRESSED MAIL IS RETURNED TO SENDER. BEFORE TEMPORARY EXPIRED, I CONTACTED FAA TO CORRECT THE ADDRESS PROB. I WAS TOLD ADDRESS CORRECTION WAS MADE AT THE TIME MY EXTENSION WAS FAXED TO ME. MORE THAN 60 DAYS HAVE PASSED SINCE CORRECTION WAS SUPPOSEDLY MADE BY FAA, AND I STILL DO NOT HAVE MY PERMANENT CERTIFICATE. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATES HE MADE MANY MORE CALLS TO OKC, RECEIVED FAX WITH EXTENSION OF TEMPORARY CERTIFICATE AGAIN. FINALLY RECEIVED PERMANENT COPY.
