Narrative:

Upon picking up ATIS which stated that takeoffs and lndgs would be conducted on runway 31, we tuned in ground and were cleared to the end of runway 31. Once we began our taxi we were asked if we would like the intersection B takeoff, that there would be 6000 ft left. We said that we would take the intersection takeoff. After runup we contacted the tower and said that we were ready to go runway 31. The tower then had us hold short for a landing fgt. The crux of the whole problem is that I had runway 31 on the mind. We were then cleared for takeoff on runway 35 (I heard runway 31). On callback I simply stated 'aircraft cleared for takeoff.' we then expedited the takeoff so not to get in the way of the second fgt on final. I remember thinking to myself -- man they are sequencing us in tight. I was then informed that we had departed the wrong runway and that I could call the tower when I got home. The need to fill out this form and the mental anguish of knowing that I will be contacted by the FAA in the next few days could be avoided by simply giving a complete callback, and you can bet your life that for the rest of mine, I will never accept another clearance without a complete callback.

Title: PVT PLT TOOK OFF FROM RWY 31 INSTEAD OF RWY 35.

Narrative: UPON PICKING UP ATIS WHICH STATED THAT TKOFS AND LNDGS WOULD BE CONDUCTED ON RWY 31, WE TUNED IN GND AND WERE CLRED TO THE END OF RWY 31. ONCE WE BEGAN OUR TAXI WE WERE ASKED IF WE WOULD LIKE THE INTXN B TKOF, THAT THERE WOULD BE 6000 FT LEFT. WE SAID THAT WE WOULD TAKE THE INTXN TKOF. AFTER RUNUP WE CONTACTED THE TWR AND SAID THAT WE WERE READY TO GO RWY 31. THE TWR THEN HAD US HOLD SHORT FOR A LNDG FGT. THE CRUX OF THE WHOLE PROB IS THAT I HAD RWY 31 ON THE MIND. WE WERE THEN CLRED FOR TKOF ON RWY 35 (I HEARD RWY 31). ON CALLBACK I SIMPLY STATED 'ACFT CLRED FOR TKOF.' WE THEN EXPEDITED THE TKOF SO NOT TO GET IN THE WAY OF THE SECOND FGT ON FINAL. I REMEMBER THINKING TO MYSELF -- MAN THEY ARE SEQUENCING US IN TIGHT. I WAS THEN INFORMED THAT WE HAD DEPARTED THE WRONG RWY AND THAT I COULD CALL THE TWR WHEN I GOT HOME. THE NEED TO FILL OUT THIS FORM AND THE MENTAL ANGUISH OF KNOWING THAT I WILL BE CONTACTED BY THE FAA IN THE NEXT FEW DAYS COULD BE AVOIDED BY SIMPLY GIVING A COMPLETE CALLBACK, AND YOU CAN BET YOUR LIFE THAT FOR THE REST OF MINE, I WILL NEVER ACCEPT ANOTHER CLRNC WITHOUT A COMPLETE CALLBACK.
