Narrative:

This entire 4-DAY trip series was marred by repeated lack of professional performance by the captain, including multiple instances of improper checklist use, radio word games with controllers and violations of company operations manual/FARS. In short, the captain was lacking in basic air discipline. Specifically, at least once each day, the captain would call for a checklist before he had done the items, get through the checklist without completing the item/give the proper response. Then he would do the item. Once he jumped from initial checks above the line, omitted initials below the line, and started through the final checks reporting 'gear down/3 green' when in fact the gear were up/locked and the panel clear. 50 percent of the flts he omitted the before start checklist. Senior capts are bad enough about omitting the before start checklist that most first officer's just do the checklist before the captain is on the plane. This captain chewed out jax approach controllers 3 times for her poor performance (which wasn't good) but he could better have spent his time on his own performance. On one leg in the bahamas, another captain jump-seated with us. They both broke sterile cockpit, talking about golf as close as short final. Also, the jump seating captain said 'it sure is warm in here' -- he was holding a map light close to my neck as a source of heat. My captain reads newspapers almost every leg, sometimes in violation of sterile cockpit, always in violation of the company manual. This captain's lack of situational awareness leads him into repeated erratic responses when a properly alert person would have routine responses. Alpa standards has alluded that I'm not the only person having these problems with this other captain. Once I reported a similar trip with this captain to our previous chief pilot. He said 'no doubt about it you are flying with the most dangerous captain in the company.' now, a yr later, neither the company nor alpa has fixed it. I'm giving up on this captain. I am looking for another job but would appreciate it if this did not get back to the company until I find some other means of supporting my family. I very much regret having to write this letter but safety requires changes. I just cannot get management to implement.

THEN HE WOULD DO THE ITEM. ONCE HE JUMPED FROM INITIAL CHKS ABOVE THE LINE, OMITTED INITIALS BELOW THE LINE, AND STARTED THROUGH THE FINAL CHKS RPTING 'GEAR DOWN/3 GREEN' WHEN IN FACT THE GEAR WERE UP/LOCKED AND THE PANEL CLR. 50 PERCENT OF THE FLTS HE OMITTED THE BEFORE START CHKLIST. SENIOR CAPTS ARE BAD ENOUGH ABOUT OMITTING THE BEFORE START CHKLIST THAT MOST FO'S JUST DO THE CHKLIST BEFORE THE CAPT IS ON THE PLANE. THIS CAPT CHEWED OUT JAX APCH CTLRS 3 TIMES FOR HER POOR PERFORMANCE (WHICH WASN'T GOOD) BUT HE COULD BETTER HAVE SPENT HIS TIME ON HIS OWN PERFORMANCE. ON ONE LEG IN THE BAHAMAS, ANOTHER CAPT JUMP-SEATED WITH US. THEY BOTH BROKE STERILE COCKPIT, TALKING ABOUT GOLF AS CLOSE AS SHORT FINAL. ALSO, THE JUMP SEATING CAPT SAID 'IT SURE IS WARM IN HERE' -- HE WAS HOLDING A MAP LIGHT CLOSE TO MY NECK AS A SOURCE OF HEAT. MY CAPT READS NEWSPAPERS ALMOST EVERY LEG, SOMETIMES IN VIOLATION OF STERILE COCKPIT, ALWAYS IN VIOLATION OF THE COMPANY MANUAL. THIS CAPT'S LACK OF SITUATIONAL AWARENESS LEADS HIM INTO REPEATED ERRATIC RESPONSES WHEN A PROPERLY ALERT PERSON WOULD HAVE ROUTINE RESPONSES. ALPA STANDARDS HAS ALLUDED THAT I'M NOT THE ONLY PERSON HAVING THESE PROBS WITH THIS OTHER CAPT. ONCE I RPTED A SIMILAR TRIP WITH THIS CAPT TO OUR PREVIOUS CHIEF PLT. HE SAID 'NO DOUBT ABOUT IT YOU ARE FLYING WITH THE MOST DANGEROUS CAPT IN THE COMPANY.' NOW, A YR LATER, NEITHER THE COMPANY NOR ALPA HAS FIXED IT. I'M GIVING UP ON THIS CAPT. I AM LOOKING FOR ANOTHER JOB BUT WOULD APPRECIATE IT IF THIS DID NOT GET BACK TO THE COMPANY UNTIL I FIND SOME OTHER MEANS OF SUPPORTING MY FAMILY. I VERY MUCH REGRET HAVING TO WRITE THIS LETTER BUT SAFETY REQUIRES CHANGES. I JUST CANNOT GET MGMNT TO IMPLEMENT.
