Narrative:

We were flight planned for cmh to ord. Left cmh en route to fwa. Thunderstorms en route and around ord area. Almost to fwa, ZAU called and said expect rerouting to pontiac and then direct ord. We then received rerte to pontiac, then ord. First officer dialed in pontiac (ptk), michigan, 111.0 off L20-21 chart. What ZAU wanted was for us to go to pontiac (pnt), illinois, 109.6 H7-8 chart. Proceeding up from southeast one can easily confuse the 2 stations. Suggest one of the stations be renamed.
