Narrative:
The captain and I had been on duty since XA00 am. Arrival into orf was 1 hour late. During approach, we forgot our approach check and I fell asleep for 15-20 seconds while hand-flying the ILS to a visual. The captain awoke me by calling my high speed of 150 KTS (normal speed is 130 KTS). Reorientation was necessary and the subsequent landing was uneventful. Contributing factors were: 1) on duty for 13 1/2 hours. 2) heat, 90-95 degrees all day with the cockpit air conditioning pack inoperative. 3) this was the 5TH leg of a 6 leg day. Factors affecting performance: FAA regulations allow 16 hour days for part 135. Our total duty for the day ended at 15 1/2 hours. An AG00 am chkin was scheduled the next day! Upon arrival back to iad, the captain dozed momentarily and also forgot to do the approach check. We had both been seated, without getting up, for 3 1/2 hours.

Narrative: THE CAPT AND I HAD BEEN ON DUTY SINCE XA00 AM. ARR INTO ORF WAS 1 HR LATE. DURING APCH, WE FORGOT OUR APCH CHK AND I FELL ASLEEP FOR 15-20 SECONDS WHILE HAND-FLYING THE ILS TO A VISUAL.
