Narrative:
On sep/thu/93 at XA30 local time at bwi, FAA inspector proceeded to conduct a line check on our flight from bwi-orf. As is customary, I presented my airman certificates to the inspector. After examining them he reported that my address was missing from the appropriate position on the certificate. I, to this point, had never noticed this. The medical certificate was issued in dec/92 and was issued that way. At this point, I discontinued any flight operation until the situation could be rectified. A temporary was issued via the company through the FAA in oklahoma city approximately 2 hours after the inspector's discovery. I did not commence flight operations until the temporary was in my hand. A duplicate has been requested. It is certainly inexcusable that I missed this discrepancy upon the issuance of my certificate, but this discrepancy was also missed by the FAA when the copy from the examiner was submitted. It was missed by the company whom I fly for when a required copy was submitted, and the air medical examiner who overlooked the proper entries. This discrepancy was also overlooked during at least 3 previous line checks/chkrides by company personnel since dec/92.

Title: ACR FO FLUNKS RAMP CHK BY ACR INSPECTOR WHEN HIS MEDICAL CERTIFICATE DOES NOT CONTAIN PLT'S ADDRESS.
Narrative: ON SEP/THU/93 AT XA30 LCL TIME AT BWI, FAA INSPECTOR PROCEEDED TO CONDUCT A LINE CHK ON OUR FLT FROM BWI-ORF. AS IS CUSTOMARY, I PRESENTED MY AIRMAN CERTIFICATES TO THE INSPECTOR. AFTER EXAMINING THEM HE RPTED THAT MY ADDRESS WAS MISSING FROM THE APPROPRIATE POS ON THE CERTIFICATE. I, TO THIS POINT, HAD NEVER NOTICED THIS. THE MEDICAL CERTIFICATE WAS ISSUED IN DEC/92 AND WAS ISSUED THAT WAY. AT THIS POINT, I DISCONTINUED ANY FLT OP UNTIL THE SIT COULD BE RECTIFIED. A TEMPORARY WAS ISSUED VIA THE COMPANY THROUGH THE FAA IN OKLAHOMA CITY APPROX 2 HRS AFTER THE INSPECTOR'S DISCOVERY. I DID NOT COMMENCE FLT OPS UNTIL THE TEMPORARY WAS IN MY HAND.
