Narrative:

On arrival to dulles, we were cleared via a radar vector to intercept the ILS course for 19L. We captured the ILS normally then with less than 10 mi to go we were cleared for the ILS 19L converging approach. I had never flown this approach before. I had to get the binder out and find the approach plate. The first one I found for 19 converging was for the right runway. I did not notice this. I tuned in the new frequency and the pilot turned right to intercept the course. The pilot noticed that the situation didn't look right and just questioned it when the tower told us we were off course. We immediately executed a go around. We then were vectored around for another approach. The tower asked us to call them when on the ground. We discussed the incident with them including changing the approach after being cleared for one and captured on it. Also, it's important for the crew members to make sure they identify the correct approach charts especially when a change in the approach occurs on short notice.

Title: MLG FLC SELECTS WRONG APCH PLATE CAUSING HDG DEV AND MISSED APCH.

Narrative: ON ARR TO DULLES, WE WERE CLRED VIA A RADAR VECTOR TO INTERCEPT THE ILS COURSE FOR 19L. WE CAPTURED THE ILS NORMALLY THEN WITH LESS THAN 10 MI TO GO WE WERE CLRED FOR THE ILS 19L CONVERGING APCH. I HAD NEVER FLOWN THIS APCH BEFORE. I HAD TO GET THE BINDER OUT AND FIND THE APCH PLATE. THE FIRST ONE I FOUND FOR 19 CONVERGING WAS FOR THE R RWY. I DID NOT NOTICE THIS. I TUNED IN THE NEW FREQ AND THE PLT TURNED R TO INTERCEPT THE COURSE. THE PLT NOTICED THAT THE SIT DIDN'T LOOK RIGHT AND JUST QUESTIONED IT WHEN THE TWR TOLD US WE WERE OFF COURSE. WE IMMEDIATELY EXECUTED A GAR. WE THEN WERE VECTORED AROUND FOR ANOTHER APCH. THE TWR ASKED US TO CALL THEM WHEN ON THE GND. WE DISCUSSED THE INCIDENT WITH THEM INCLUDING CHANGING THE APCH AFTER BEING CLRED FOR ONE AND CAPTURED ON IT. ALSO, IT'S IMPORTANT FOR THE CREW MEMBERS TO MAKE SURE THEY IDENT THE CORRECT APCH CHARTS ESPECIALLY WHEN A CHANGE IN THE APCH OCCURS ON SHORT NOTICE.
