Narrative:

I took a part 91 flight in city 'a' then drove back to city 'B' and did a 3-HR night shift. On checking the schedule I noted that I was back on day shift at XA00. I told everyone at the hospital that I was not on duty and would only fly in an extreme emergency and would require a 10-hour rest prior to my next duty. I would call in another pilot if I had to fly. I did not fly on the night shift, but I did stay at the hospital till after beginning of next day. I then also reported for my duty shift at XA00 the next day. The part 91 flight I flew was a change of schedule and I did not foresee the conflict of the night then day shift which would not give the required 10 hours rest between duty shifts.
